this inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was imitated on 06/03/21 and concluded on 06/07/2021. The self-reported incident was received by the department regarding forbidden actions. The director was contacted by pone to conduct the investigation. The inspector received from the director information needed to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.

Standard #: 22VAC40-185-410-1 Description: Based on video review and interviews, center staff roughly handled a child.
Evidence:
1. Staff #1 was seen on the video reaching for child #1's arm and grasps her elbow area and pulls the child towards the rug area. Staff #1 then picks up the child under the arm/armpit area, then sits the child beside the video cabinet. 2. Photos of the child's arm shows fingernail marks. 3. Child #1 reported to staff #2 that staff #1 had caused the marks on her arm.Plan of Correction: 1. Staff #1 has been assigned to administrative duties during the investigation. 2. Staff have been retrained on age-appropriate activities. 3. A staff person has been assigned to this center from a different center.
