Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on June 7, 2021 and concluded on June 22, 2021. The director was contacted by telephone to initiate the inspection at 10:00 a.m. The inspector conducted a virtual inspection via video conferencing with the director. There were 40 children present under the care and supervision of 6 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed five children?s records and three staff records submitted by the facility to ensure documentation was complete

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of documentation, the center failed to obtain documentation of a physical exam for each child within 30 days of enrollment. Evidence: Child #3, enrolled 4/29/21, did not have a physical on file.

Plan of Correction: We followed up with the parent several times and we are expecting it to be submitted by June 24, 2021.

Standard #: 22VAC40-185-260-B
Description: Based on review of documentation and interview with staff, it was determined that the center did not obtain an annual approval from the health department. Evidence: The most recent health inspection was 5/8/2019.

Plan of Correction: Left a message with the Dept of Environmental Health to schedule an inspection. I attempted to locate an email address without success. I will call weekly until a virtual inspection is scheduled and completed.

Standard #: 22VAC40-185-340-A
Description: Based on observation, it was determined that staff did not ensure the care, protection and guidance of all children in care. Evidence: On June 7, 2021, Child A, age 3, was observed climbing on top of a bookshelf in the back of the classroom. The classroom was not in the required staff to children ratio of 1 staff for every 10 children and the staff member was actively supervising 11 other children in the front of the classroom area.

Plan of Correction: We added a second staff member to the classroom on 6/10/2021

Standard #: 22VAC40-185-350-E-4
Description: Based on observation, review of documentation and interview with staff, it was determined that the center failed to adhere to the required ratio of 1 staff for every 10 children ages three years old to five years old. Evidence: On June 7, 2021, the "ducks" classroom was observed with 12 children ages 3 years to 4 years old under the supervision of 1 staff. Documentation and interviews revealed that the week of May 24, 2021 there were 12 children with 1 staff, the week of 6/1 there were 12 children with 1 staff, and the week of 6/7 there were up to 14 children with 1 staff member.

Plan of Correction: Due to staff emergency, we were short staffed. At the time of inspection we were also in the process of hiring. Since then we hired one additional staff on 6/10/2021 and another staff member on 6/14/2021.
