Standard #: 22VAC40-665-580-H Description: Based on a review of four staff records and interview, the vendor did not ensure three staff who work directly with children completed the department's health and safety update course annually.
Evidence: 1) The most recent Annual Health and Safety Update Training in the record for Staff #2, hired on 08/19/19, was completed on 08/15/19. The training was not completed annually in 2020.
2) The most recent Annual Health and Safety Update Training in the record for Staff #3, hired on 11/09/18, was completed on 04/19/19. The training was not completed annually in 2020 or 2021.
3) The most recent Annual Health and Safety Update Training in the record for Staff #4, hired on 10/30/19, was completed on 10/30/19. The training was not completed annually in 2020.
4) During interview, a member of management acknowledged Staff #2, Staff #3, and Staff #4 did not complete the Annual Health and Safety Training annually.Plan of Correction: Per the Vendor: "All staff have completed the course and we will continue do it yearly. The director will check these trainings yearly."
