This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/3/2021 and concluded on 6/25/2021. A self-reported incident was received by the department regarding allegations in the area of programs. The program specialist was contacted by telephone to conduct the investigation. The inspector emailed the program specialist a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-400-A Description: Based on interview statement and review of documentation, it was determined that a staff person did not interact with a child in a way that promoted their emotional and social well-being and did not respect their personal privacy.
Evidence:
1. On 4/29/2021, Staff #1 pulled Child #1's (7 years old) skirt down twice, while the child was using the outdoor playground equipment.
2. On 6/24/2021, Staff #2 viewed a video recording of the incident, they verified that Staff #1 pulled Child#1's skirt down on two separate occasions, while the child had both of her hands up and was holding on to the playground equipment.Plan of Correction: Upon receipt of learning this information the employee was immediately terminated, and CPS and Licensing were notified immediately.
