Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on07/19/2021 and concluded on 07/28/2021. The director was contacted by telephone to initiate the inspection. There were 104 children present and 19 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on a record review and interview, the center did not ensure the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
The center did not ensure the activities and services were maintained in accordance with their medication policy. Child #5 had an authorization to administer an emergency medication that had expired on 02/06/21. The Director confirmed a new authorization had not been obtained. The center's policy states, "If the prescribed duration of the medication exceeds 10 business days or the prescription changes in any way, The Prescription Medication Consent Form must be reauthorized by the parent."

Plan of Correction: School management will ensure that all medication authorization forms will be reviewed and set up on a tracking system that ensures that we request expiring authorization updates 30 dates in advance. This tracking form will be reviewed by the DM during quarterly licensing reviews. This particular child?s form was sent home the same day of the visit for updating from the child?s physician.
