This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A monitoring inspection was initiated on 05/25/2021 and concluded on 07/29/2021. A self-reported incident was received by the department regarding allegations in the areas of supervision. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, and staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22VAC40-185-340-F Description: Based on staff interviews, the facility did not ensure that all children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence: 1) At approximately 10:00 a.m. on May 21, 2021, Staff #1 walked Child #1 (age 5) to their classroom. Staff #1 reported she called out the child's name, left the child in the classroom, and walked back to the lobby. When Staff #1 was walking to the lobby, she passed Staff #2 in the hallway. Staff #2 went to the classroom and found that Child #1 was alone and unsupervised. Staff #1 acknowledged she did not check if there was anyone in the classroom when she left Child #1. It is estimated that Child #1 was out of sight and sound supervision of staff for approximately 1 to 2 minutes.Plan of Correction: Per the Center: "When a teacher is leaving a classroom, always turn off the light. When a staff member drop [sic] a child in a classroom, eye contact between the 2 adults must be made."
