Technical Assistance:
1. Reviewed recordkeeping requirements.
2. All dates on records are to include the year.
3. Discussed diaper changing procedures.
4. Reviewed/trained vendor on new subsidy requirements.
5. Forms provided: new emergency plan, attendance, and emergency contact.

Comments:
10:32am-12:10pm Annual unannounced subsidy inspection completed. No children were present today. Met with vendor to review subsidy regulations. The childcare areas of the home were found to be clean, safeguarded appropriately, and well supplied with toys + equipment for the children. The vendor is currently certified in First Aid/CPR. Record keeping and health non-compliances found.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on records reviewed and interview conducted, it was determined that a written hard copy of daily attendance that documents the arrival + departure of each child as it occurs has not been maintained. Evidence: There were no written attendance records prior to 10/1/19. The vendor said she gives them to the parent, and does not keep a copy for her records.

Plan of Correction: The vendor will write attendance for each child and keep the records for 5 years after termination of service.

Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 3 of 3 children's records did not include required information. Evidence: Child #3, #4, and #5 - Address for the emergency contact person listed.

Plan of Correction: The vendor will have the parent write this on the form. Moving forward, the vendor will use the updated emergency contact form.

Standard #: 22VAC40-665-210-B
Description: Based on records reviewed, it was determined that 2 of 3 children's records did not include immunization every 6 months until the age of 2 years old.

Evidence:

1. Child #4 - Immunizations were dated December 2018, when the child was 21 months old.

2. Child #5 - Immunizations were dated February 2019, when the child was 6 months old.

Plan of Correction: The vendor will request updated immunization records.

Standard #: 22VAC40-665-370-E-3
Description: Based on interview conducted, it was determined that the diaper changing surface has not been cleaned with soap + water and sanitized with bleach + water after each use. Evidence: The vendor said she sprays bleach + water on the pad and wipes it with bleach wipes.

Plan of Correction: The vendor will clean + sanitize the diaper pad.
