Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on April 9, 2021 and concluded on June 11, 2021 . A complaint was received by the department regarding allegations of forbidden actions. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation and interviews were conducted.

The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law and violations were not issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on interviews the center failed to comply with the center's own policies and procedures that are required by the standards.
Evidence: 1. The center's policy states "staff will maintain safety practice for infants, toddlers, and preschool children in classrooms and outdoors. Children should be directly supervised by sight and sound at all times. Staff should be able to hear and see all children."
2. During an interview with outside investigators, Staff admitted to leaving the children in the 2 year old classroom for approximately 10 minutes. The children were left with an individual that was neither staff nor a volunteer.
3. During an interview with the inspector, Staff confirmed she left the children in the 2 year old classroom with an individual that was neither staff nor a volunteer.
4. During an interview, Administration confirmed that Staff left children with an individual that was neither staff nor a volunteer. Administration stated she is was not aware that individual couldn't be left alone with children.

Plan of Correction: The individual became a volunteer as of May 21, 2018, I have a file for her which includes the sworn statement, the Virginia record and FBI screening, the central registry background from the Department of Social Services, along with her CPR/First Aid and her tuberculosis screening. Her CPR/First Aid training and tuberculosis screening needs to be updated and she is currently working to get this done. Her file is missing the references and the orientation. She volunteers periodically at the daycare, however I will ensure she has what is needed to become an employee.
