Technical Assistance:
During the virtual inspection of the center a total of 3 rooms were observed. The children were engaged in both group and center based activities. Good ratios in all rooms.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 07/28/2021. The Administrative Assistant was contacted by telephone and a virtual inspection was conducted. There were 31 children present, ranging from 3 months to 5 years in age, with 6 classroom staff supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 5 staff records were reviewed.

Information gathered during the inspection determined that there were non-compliances with the applicable standards or law. These violations were documented on a Violation Notice issued to the program.

Standard #: 22VAC40-185-160-C
Description: Based on review, 3 of 5 staff files indicated that it has been more than 2 years since the tuberculosis (TB) documentation on file at the center has been updated.

Evidence -
1. On the date of inspection (07/28/2021), the most recent TB documentation on file for Staff #3 was dated - 07/19/2019.
2. The most recent TB documentation on file for Staff #4 was dated - 07/09/2019.
3. The most recent TB documentation on file for Staff #5 was dated - 06/26/2019.

Plan of Correction: All expired TB documentation shall be updated. Staff who have been reminded, shall be directed again to fulfill this requirement. All updated documentation shall be placed in the applicable staff file.

Standard #: 22VAC40-185-60-A
Description: Based on review, 1 of 5 children files determined that not all of the required documentation was contained within each child's record.

Evidence -
1. On the date of inspection (07/28/2021), the health information (as required by 22 VAC40-185-130 through 22 VAC40-185-150) was not on file. Documentation of both the immunizations and a physical for Child #3 were not available for review.
2. The start date for Child #3 was listed as 06/21/2021.

Plan of Correction: The parent stated that an appoint has been scheduled with the child's doctor. Once this documentation has been obtained, it shall then be placed on file.
