This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on August 18, 2021 and concluded on August 23,2021. The director was contacted by telephone and a virtual inspection was conducted. There were 20 children present, ranging in ages from 10 to 14, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-191-40-D-1-B Description: Based on record review and Program Coordinator interviews, the licensee did not ensure to obtain a Search of the Central Registry for all employees of a licensed, facility who is involved in the day-to-day operations or who is alone with, in control of, or supervising one or more children before 30 days of employment at the facility ends.
Evidence: The record for staff #2, date of hire June 6, 2021 did not include the findings from a Virginia Search of the Central Registry. The Program Coordinator confirmed the check were not available for review.Plan of Correction: HR has resubmitted a CPS check request and is waiting results. The staff will not work until it is received.
