Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on August 25, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 15 children present, ranging in ages from 9 months to 6 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children's records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on the review of three staff records and confirmation from the director, the center failed to obtain documentation of a negative tuberculosis (TB) screening for two new staff members.

Evidence:
1. The record for Staff Member B did not contain the results of a negative TB screening or test. Date of hire for Staff Member B was 07/19/2021.
2. The record for Staff Member C did not contain the results of a negative TB screening or test. Date of hire for Staff Member C was 02/10/2021.
3. The director confirmed that the staff members had not gotten their TB screenings.

Plan of Correction: Both staff members are scheduled to receive their TB screenings on 09/02/2021. In the future, the director will ensure that TB screenings are obtained at the time of employment.

Standard #: 22VAC40-185-240-A
Description: Based on the review of three staff records and discussion with the provider, one staff member did not receive the required training by the end of their first day of assuming job responsibilities.

Evidence:
1. The record of Staff Member C did not contain documentation that orientation training had been completed.
2. Staff Member C was a rehire. and the director had documentation that the training had been completed on the original hire date of 12/19/2019. The director confirmed that orientation training was not completed when the staff was rehired on 02/10/2021.

Plan of Correction: The director will complete the orientation training with Staff Member C as soon as possible.
