An Initial inspection was initiated on August 16, 2021 and concluded on August 30, 2021 to determine compliance with licensing standards. The inspection was conducted on site with the owner. The facility plans to provide care to children birth through age 12 years. Measurements of the facility were calculated along with review of the certificate of occupancy to determine a capacity of 89 children. Classrooms are equipped with a variety of age and stage appropriate supplies and materials. Children will provide their own lunch and the center will provide breakfast and an afternoon snack. Prescription medications will be administered with proper authorization. Staff have MAT certification. Standards related to supervision and program could not be determined as the facility is not yet fully operational. There were no children enrolled at the time of the initial inspection. The owner is known to the Department and was knowledgeable of the standards. A conditional license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-40-E Description: Based on review of documentation, the center did not ensure the operational responsibilities of the licensee included the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
A. Information required to be provided in writing to parents prior to the child's first day of
attendance did not include the following: 1. The center's policies for the arrival and departure of children, including procedures for verifying that only persons authorized by the parent are allowed to pick up the child, picking up children after closing, when a child is not picked up for emergency situations including but not limited to inclement weather or natural disasters; 2. Description of established lines of authority for staff; 3. Policy for reporting child abuse and neglect as required by The Code of Virginia; 4. Discipline policies including acceptable and unacceptable discipline measures.
B. The following required written policies for staff were not addressed in the policies and procedure manual; 1. Procedures for Children Arriving Late; 2. Procedures for Absent Child (when child attends another program); 3. Procedures for Identifying Where Children Are at All Times; 4. Procedures for Ill, and Injured Children; 5. Procedures for Medication Administration; 6. Safety Policies for Swimming/Wading; 7. Job responsibilities and to whom they report; and 8. Confidential treatment of personal information about children in care and their families.Plan of Correction: Per the applicant: I will update and add the required policies and procedures o out handbooks.
Standard #: 22VAC40-185-270-A Description: Based on observation, the facility did not ensure all areas of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. A section of the wooden privacy fence running along the side of the building leading to the playground had a gap between the pickets that exposed four nails. The gap and exposed nails posed a risk of entrapment and snagging the skin of a child in care.
2. The poured in place resilient surfacing had several holes where previous equipment had been installed. The holes had been filled in however the repairs created an uneven surface posing a tripping hazard.Plan of Correction: Per the Owner: 1. We have a handy man scheduled to repair and replace the section of the fence where the gap is exposing nails. 2. We have blocked off the area where the poured in place resilient surfacing is located. Children will not play in that area but rather in the mulch area where the playground equipment is located. We will look into having someone evaluate the surface for replacement or repairs.
