Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 09/002/2021 and concluded on 09/07/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 41 children present, ranging in ages from 5 months to 4 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
The record for Child #3 did not contain work phone number of each parent who has custody

Plan of Correction: We will contact the parents to request the missing information.

Standard #: 22VAC40-185-540-E
Description: Based on review of emergency supplies, not all required emergency supplies were available.
Evidence:
1) On the date of inspection, one working, battery-operated radio was not available for review. The center had a "hand crank" radio which does not meet the requirement.

Plan of Correction: We will order a battery-operated radio.
