Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 08/27/2021 and concluded on 08/31/2021. There were 78 children present with 5 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined noncompliance with subsidy regulations and the violation is documented on the violation notice issued to the facility.

Description: Based on record review, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

1. There is no written allergy care plan on file for child 1.
a. Child 1 has a diagnosed food allergy according to written information in child 1's enrollment record and physical exam.

Plan of Correction: Going forward, a written allergy care plan will be obtained prior to enrollment for each child with a diagnosed food allergy.
