Comments:
An unannounced complaint inspection was conducted in response to a complaint that was received by licensing on 09/13/2021 regarding allegations in the areas of building maintenance, play areas, and equipment and materials. The inspection was initiated and completed on 09/14/2021. On 09/14/2021, the inspector was on site from 9:55 am to 10:35 am.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. If you have any questions about this inspection, please contact the licensing inspector at (540) 430-0384.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation, the facility did not ensure all areas and equipment of the center were maintained in a clean, safe and operable condition.

Evidence:
1. A wooden bench on the playground had a board that was broken with splintered wood. Rusting was observed on the fence. The fence had areas where the metal wire had come loose and could entangle clothing or snag skin. One area of the fence had come loose from the supports and posed a risk of entrapment. Two toy riding cars were observed to have cracked and splintered pieces.

Plan of Correction: Toys are being evaluated and removed from the playground immediately. The fence will be reattached to its supports and will also have metal wires fixed. The rust on the fence will be painted with rust-oleum. The wooden bench will be removed or repaired.

Complaint related: No
Description: Based on observation, the center did not ensure that the playground equipment provided to the children contained enough resilient surfacing to comply with minimum safety standards.

Evidence:
1. The playground contained a climbing structure with a slide which required a minimum depth of 6 inches. Three depth samples were taken that all measured under 2 inches.

Plan of Correction: use of the climbing structures ceased for the moment. Mulch has been ordered and delayed due to trucking. Shipment should arrive in 3-4 weeks.

Complaint related: No
Description: Based on observation, the center did not ensure that S-hooks where provided were not open more than the thickness of a penny.

Evidence:
1. S-hooks were observed on the tops and bottoms of the swings. The S-hook openings exceeded the thickness of a penny.

Plan of Correction: Use of the swings has ceased for the moment. The s-hooks will be replaced or repaired to meet the standards.
