Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated and concluded on September 15, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 51children present , ranging in ages from 3 months to 5 years old, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, required records were not kept for each staff person.
Evidence:
1. Staff #4's record (Date of hire-7/12/21) did not contain proof of education.

Plan of Correction: Proof of education was emailed to the center by the staff person, but documentation was not in the record at the time of inspection. A copy of proof of education will be obtained and put into the staff person's record.

Standard #: 22VAC40-185-440-B
Description: Based on observation and interview, cots in two of six classrooms were not identified for use by a specific child.
Evidence:
1. Cots in the Preschool and Pre-K classrooms were not identified for use by a specific child.
2. Staff #5 confirmed that the cots were not labelled for the Preschool and Pre-K classrooms.

Plan of Correction: The cots will be labelled to identify specific use for each child in the Preschool and Pre-K classrooms.

Standard #: 22VAC40-185-510-C
Description: Based on observation and record review, the center did not ensure that the center's medication policy included methods to prevent use of outdated medication.
Evidence:
1. Emergency medication for Child #6 was observed during the time of the inspection with an expiration date of 03/23/2021.
2. The medication policy was reviewed and stated the following: Medications must have an expiration date. If left on site more than 7 days after the end of the course of treatment and expired, medication must be disposed of.

Plan of Correction: The authorization form was dated 03/17/2021. The center will return the emergency medication to the parent immediately and have the parents to supply an updated medication.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the menu listing foods to be served for meals and snacks during the current one week period was not dated.
Evidence:
1. The school menu listing foods to be served for meals and snacks during the week was labelled as "Week 3" and not dated for the current month, day or year.

Plan of Correction: The menus were corrected during the time of inspection to reflect the dates.
