Comments:
The licensing inspector conducted an announced initial inspection from 10:10 am - 12:45 pm. Areas to be used by the children in care inside and outside the center were inspected. All areas of the standards were reviewed but compliance for some standards could not be determined because the center is not fully operational and there are no children in care. One staff record was reviewed.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, the provider failed to maintain the outside area of the center in a safe condition.
Evidence:
1. The inspector observed the outside play area. There is a sign pole close to the outer fence that is rusted. There is a pipe stub beside the sign pole that is protruding from the ground creating a tripping hazard. There is open space under the fence bar on the side of the playground next to a culvert. The open space measures 3 to 4 inches and would allow a child's foot to go through and get stuck under the fence.

Plan of Correction: The provider will paint the sign post and place a caution cone over the pipe stub. The provider will purchase and attach a board over the open space at the bottom of the fence under the fence bar. Once the work is completed, the provider will send pictures to the licensing office to confirm these area are no longer unsafe.

Standard #: 22VAC40-185-290-3
Description: Based on observations, the provider failed to have protective covers in all electrical outlets.
Evidence:
1. The inspector observed the inside of the center. There were two outlets without protective covers in the preschool classroom next to the book shelf. There were two outlets in school-age classroom without protective covers next to the table (preschoolers have access to area). There was one outlet without a protective cover in the 2's classroom and two additional outlets without protective covers in the hallway next to the exit to the outside play area.

Plan of Correction: The provider will purchase additional protective outlet covers today and place in the outlets. The provider will monitor to ensure all outlets remain covered.
