63.2(17) License & Registration Procedures

Comments:
A monitoring inspection was initiated on 09/08/2021 and concluded on 09/20/2021. The director was contacted by telephone to initiate the inspection. There were 59 children present and 18 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.


Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-510-G
Description: Based on review of two medications, the center failed to ensure the prescription medication had the prescription label.

Evidence:
1. Child # 2 medication that was for a food allergy did not have the pharmacy prescription label on the medication.

Plan of Correction: The acting director will have the parent to obtain a new label and will monitor all medications.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of four staff's records, the center failed to ensure that all repeat background checks were completed within the 5 year time frame.

Evidence:
1. Staff # 1 CPS Background Check was completed 10/15/2015 and the repeat CPS Background check was completed on 1/15/2021. The requirement is for the repeat check to be completed within 5 years.

Plan of Correction: In the future all repeat background checks will be completed on time within the 5 year time frame.
