Technical Assistance:
The following topics were discussed with the program director:
- placement of changing table and sink, using service side of changing table with easy access to sanitizing chemicals underneath.
- menu - peanut butter as protein needs additional protein per serving
corn dogs must be cut lengthwise into small pieces for children 3 years and under.
- clear documentation of hire date and job title in staff records

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A renewal inspection was initiated on 09/16/2021 and concluded on 09/16/2021. The director was contacted by telephone to initiate the inspection. There were 12 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-240-D-5
Description: Based upon review of records and staff interview, the facility has not ensured that there is always at least one staff member on duty who has obtained within the last three years instruction in performing daily health observation of the children.
1. The only documentation of daily health screening training in the staff records was in the record of staff 1.
2. Staff 1 stated that she is only at the facility on Mondays, Tuesdays and half of Fridays. Staff 1 acknowledged that there is no staff person with daily health observation training on duty when she is not there.

Plan of Correction: The facility responded with the following:
Director will have staff take a DHO training.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas and equipment of the center, inside and outside, are maintained to be in a safe condition.
1. Two areas of the artificial lawn turf carpet on the playground where children were observed playing, had come loose and folded into long raised areas creating tripping hazards. Staff 1 acknowledged the hazard and asked staff to begin pulling the carpeting back into place.
2. The black electrical cord to the portable hand washing station adjacent to the changing table was pulled away from the handwashing station, creating a potential tripping hazard. Staff 2 was observed carrying an infant from one side of the changing table to the other, stepping over the cord.

Plan of Correction: The facility responded with the following:
Artificial lawn was corrected while on virtual call with inspector.
Electrical cord was corrected the same day.

Description: Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
After using a spray bottle of sanitizer, staff 2 returned the spray bottle to the cabinet under the changing table and walked away without locking the cabinet.

Plan of Correction: The facility responded with the following:
Have staff take a diaper changing training.

Description: Based upon observation, the facility has not ensured that staff wash their hands and the hands of the child after diaper changing..
1. Staff 2 did not wash her hands with soap and running water after a diaper change. Staff 2 was reminded by staff 1 after which staff 2 walked across the infant room to a bathroom to wash hands, not using the sink immediately adjacent to the changing table.
2. Staff 2 did not wash the hands of the infant whose diaper was changed until being reminded by the inspector and staff 1.

Standard #: 22VAC40-185-500-B
Description: Based upon observation, the facility has not ensured that required diaper changing procedures are followed.
1. Staff 2 was observed placing a soiled diaper in a bag, walking across the infant room with the bag to an exterior door and placing the bag outside. The diaper was not disposed of in the leak proof or plastic lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
2. Staff 2 was observed using her hands to open the covered, step on diaper pail to dispose of paper towels.
3. Staff 2 did not clean and sanitize the changing table after changing an infant's diaper. After she had walked away from the changing table, staff 1 reminded her to clean and sanitize the table. After being reminded, staff 2 sprayed the changing table with sanitizer, however the table was not first cleaned with soap and water.

Plan of Correction: The facility responded with the following:

Description: Based on observation, the facility has not ensured that prepared infant formula is dated.
A prepared bottle of infant formula was sitting on a shelf in the infant space. The bottle was not dated.
