Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A subsidy inspection was initiated on 09/16/2021 and concluded on 09/16/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based upon staff interview, the facility has not ensured that they maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions that is dated and kept confidential in each room or area where children are present.
Evidence:
Staff 3 stated that there is currently a child enrolled for a sensitivity to milk, however there was not a written and dated list available to all staff in the room where the child is in care.

Plan of Correction: The provider responded with the following:
A list of those children with diagnosed food allergies, sensitivities and dietary restrictions will be created, dated and mantined in a confidential fashion in the classrooms. All staff will be made aware of this list,

Standard #: 22VAC40-665-540-A
Description: Based upon review of two staff records, the facility has not ensured that staff have been evaluated by a health professional and have been issued a statement that the individual is determined to be free of communicable tuberculosis (TB). The statement and documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
The record provided for staff 1 indicated a hire date of 8/31/2021. The tuberculosis screening on record is dated 9/13/2021.

Plan of Correction: The provider responded with the following:
In the future, tuberculosis screenings for new staff will be obtained within 30 days prior to hire/employment.
