An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today from 12:30pm to 3:15pm in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Two staff records were reviewed for documentation of the mandatory Virginia Preservice Training, Health & Safety Update training, and current certification in First Aid and CPR. There are children currently enrolled with diagnosed food allergies and their records were reviewed for allergy care plans. The last lockdown drill was documented on 5/26/21. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/21/21. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.
Standard #: 22VAC40-665-500-E Description: Based on interview, the center did not ensure that all staff who work with children were informed of children's allergies. Evidence: In the Schoolage classroom the Licensing Inspector asked staff if there were any children with allergies in the classroom. A staff member stated "No, I don't think so." and checked the posted allergy list in the classroom. The posted allergy list contained documentation that Child A, who is in the Schoolage class, is allergic to nuts. Plan of Correction: Staff was aware, it was staff who posted the allergy list.
Standard #: 22VAC40-665-510-B Description: Based on documentation review and interview, the center did not ensure that for each group of children a written hard copy record of daily attendance was maintained that documented the arrival and departure of each child as it occured. Evidence: Approximately 10-15 minutes after the Schoolage children arrived from public school on 9/15/21, the Licensing Inspector (LI) went into the Schoolage classroom and requested the attendance. The staff member reported that the attendance was still in the hallway and went to retrieve it. The staff member further reported that the classroom attendance had not been completed yet. The LI asked the staff member to complete the attendance and the staff member did as requested. Upon review, the attendance documented 21 children in attendance while only 18 children were present. The LI asked the staff member why the attendance did not match the children present and the staff member stated that some children had already left for the day. Plan of Correction: Attendance was taken immediately in the hallway of all school age children as they arrived from the van and buses on separate van rosters and bus rosters as well as the entire school attendance roster. Staff had not yet taken attendance on the Schoolage classroom roster. All staff have been retrained on classroom attendance policy and how to document when children are checked in " / " and out " X ".
Standard #: 22VAC40-665-520-B-11 Description: Based on documentation review and interview, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The allergy list observed on 9/15/21 indicated that Child A is allergic to nuts. 2. The physical on record for Child A indicated that Child A is allergic to nuts. 3. There was no allergy care plan on record for Child A. Plan of Correction: A form has been given to the parents for the doctor to fill out.
