63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on September 21, 2021 and concluded on September 22, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 87 children present, ranging in ages from 6 weeks to age 5, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-550-D
Description: Based on record review and staff interviews ,the licensee did not ensure the center shall implement a monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence: The center did not have documentation of a completed fire evacuation drill for January 2021.

Plan of Correction: Make sure all drills are documented properly.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and staff interviews, the licensee did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: The following records did not include a sex offender registry for the states outside of VA where the staff have lived within the last five years. The sex offender registry are to be obtained prior to their first day of employment.
1) Staff #1, date of hire 8-1-2,1 did not include a sex offender registry from Georgia.
2) Staff #2, date of hire 6-1-21, did not include a sex offender registry from New York.
3) Staff #5, date of hire 5-3-21, did not include a sex offender registry from Georgia.
Staff #1, #2 and #5 were all working on September 21, 2021.

Plan of Correction: We will use the Interstate Child Care Background check contact list for persons from out of state.
