Technical Assistance:
Training provided : Updated emergency planning Annual 3 hour Subsidy provider training due March 1, 2020 Safe sleep practices for children over 1 ***Every child in care is required to have information on record. Even children in care just for the day. ***Every child in care is required to be documented at time of arrival into care and time out on attendance form.

Comments:
An unannounced SHSI inspection was conducted today from 12:15 pm-1:40pm. There were 4 children ( 7 points) in care at the time of the inspection directly supervised by the vendor. The children were observed sleeping during nap time and at parent pick up. Vendor information/ child information that was available/ household member information was reviewed. Required emergency preparedness planning, supplies and drills were also inspected. If there are any questions regarding this inspection please contact kelly.savittiere@dss.virginia.gov

Violations:
Standard #: 22VAC4-665-340-G
Description: Based upon observation, cots,rest mats or beds were not provided for children over 12 months of age. Evidence: Upon inspectors arrival 2 children were found sleeping in rockers and not provided a cot, rest mat or bed as required.

Plan of Correction: They dont normally come. ( sic.) They were moved during inspection.

Standard #: 22VAC40-665-160-B
Description: Based upon record review and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There were 4 children in care at the time of the inspection and there was no hard copy of written attendance documenting their arrival into care today.

Plan of Correction: I will do. ( vendor's son translated over telephone) . Inspector helped create list at time of inspection.

Standard #: 22VAC40-665-170-A
Description: Based upon information reviewed and interview, required child information was incomplete. Evidence: Child A and Child B in care did not have any record of information.

Plan of Correction: They only come today. ( Translated by son over telephone) Every child in care needs to have all required information.

Standard #: 22VAC40-665-240-A
Description: Based upon observation, areas of the family day home were not maintained in safe condition. Evidence: There was a queen size mattress propped up against the wall in the children's play space that could potentially be heavy and unstable.

Plan of Correction: They are my daughter's. She is moving soon.

Standard #: 22VAC40-665-380-H
Description: Based upon observation, medication for household members was not kept in a locked place using a safe locking method to prevent access as required. Evidence: Household member medication was located in the living room under the TV within reach of children in care.

Plan of Correction: Medication was moved during inspection.

Standard #: 22VAC40-665-410-B
Description: Based upon record review and interview, vendor did not maintain record of practice drills as required. Evidence: The last drill on record was 2/2019. There was no documentation for drills completed for 3/2019, 4/2019, 5/2019, 6/2019,7/2019, 8/2019.

Plan of Correction: I will find paper.
