Technical Assistance:
Technical assistance was provided regarding TB tests/screenings.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on 9/20/2021 and concluded on 9/22/2021. The site director was contacted by telephone and a virtual inspection was conducted. There were 33 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not obtain the results of a Virginia Central Registry of Child Abuse and Neglect search within 30 days for all staff.

Evidence: Staff #1 (Date of Hire: 8/12/2021) does not have Virginia Central Registry results on file.

Plan of Correction: Staff #1 will obtain a Central Registry of Child Abuse and Neglect.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and staff interview, the center did not obtain results of a criminal history record information check, a sex offender registry check for, or a criminal background check from all states where staff have resided in the last 5 years.

Evidence: Staff #1 indicated on a Sworn Disclosure Statement dated 8/12/2021 that they have lived in the state of Michigan within the last 5 years. The Michigan criminal history background check, Michigan sex offender registry search results, and the Michigan central registry search results were not on file. Staff #2 indicated on a Sworn Disclosure Statement dated 12/10/2019 that they have lived in the state of Washington within the last 5 years. The Washington criminal history background check, Washington sex offender registry search results, and the Washington central registry search results were not on file.

Plan of Correction: Staff #1, #2 will obtain criminal background check, sex offender registry check, and criminal background check.
