Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on September 20, 2021 and concluded on September 24, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 85 children present, ranging in ages from two years to six years of age, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and ten staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of staff files, the center failed to obtain documentation of a negative tuberculosis screening within 12 months prior to hire or within 21 days after employment.

Evidence:
1. Staff #1's tuberculosis screening was dated 08/18/2021. Date of employment for Staff #1 was 04/19/2021.
2. Staff #4's tuberculosis screening was dated 07/09/2020, more than 12 months prior to hire. Date of employment for Staff #4 was 08/23/2021.
3. Administration verified dates of tuberculosis screenings, and date of hire for both staff.

Plan of Correction: Moving forward all staff will be required to have tuberculosis screening completed as part of the on-boarding process.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children records, the center failed to ensure that all children records contained the required information.

Evidence:
1. Child #3's file did not contain the address for both emergency contacts listed.
2. Administration verified that the address's were not listed for both emergency contacts.

Plan of Correction: The child had moved from out-of-state and at the time of relocation the address were unknown. It was an oversight on administration not to follow-up with the parents and obtain the emergency contact addresses. Administration will contact parents at the beginning of the week and request the emergency contact addresses.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of staff records, the center failed to complete an updated central registry finding every five years.

Evidence:
1. Staff #5's central registry finding was dated 04/22/2016. An updated central registry should have been completed by 04/22/2021.
2. Administration verified that an updated central registry finding had not been completed.

Plan of Correction: In the spreadsheet used to track required updates there was an error on when the updated was needed by. Administration will have the new employee complete a central registry finding immediately. Administration will send proof to the Licensing Inspector with 10 days that the central finding has been completed or sent out.
