Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A monitoring inspection was initiated on 9/22/2021 and concluded on 9/22/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 75 children present, ranging in ages from 5 to 12 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-240-A
Description: Based on review of five staff records, the facility failed to have documentation that each staff received orientation training by the end of their first day of assuming job responsibilities. Evidence: Staff A (date of hire 3/12/2021) and Staff B (date of hire 6/27/2021) had documentation of orientation training, but it was not dated.

Plan of Correction: All staff received orientation training again on 9/23/2021.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff files, the facility failed to have a completed search of the central registry (CPS check) within 30 days of employment for each staff. Evidence: Staff 1 was hired by the agency 7/6/2020 and worked in a licensed child day center from 3/12/2021 to present. There is no documentation of a search of the central registry for Staff A on file.

Plan of Correction: If the search of the central registry can not be located, a new one will be submitted.
