Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.

A renewal inspection was initiated on 9/28/2021 and concluded on 10/6/2021. The director was contacted by telephone to initiate the inspection. There were 2 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based upon review of two staff records and staff interview, the facility has not ensured that tuberculosis screenings for staff are obtained at least every two years.
Evidence:
1. The most recent tuberculosis screening in the record provided for staff 1 is dated 9/18/2019.
2. The most recent tuberculosis screening in the record provided for staff 2 is dated 9/13/2019.
3. Staff 1 verified the dates of the tuberculosis screenings for staff 1 and staff 2.

Plan of Correction: The facility responded with the following:
Staff 1 and staff 2 will obtain new tuberculosis screenings. In the future, staff will be sent for new tuberculosis screenings prior to two years from the most recent screenings.

Standard #: 22VAC40-185-260-A
Description: Based upon review of documents, the facility has not ensured that an annual fire inspection from the appropriate fire official is obtained.
Evidence:
The most recent fire department inspection provided and posted was on 1/3/2020.

Plan of Correction: The facility responded with the following:
We will contact the local fire department to schedule an inspection as soon as possible. If an inspection cannot be scheduled, we will document our efforts to obtain an inspection.
