This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on September 30, 2021 and concluded on October 12, 2021 to investigate a self reported incident of alleged forbidden action of physical discipline and abuse and neglect. The director was contacted by telephone and a virtual and in-person inspection was conducted. Staff interviews were conducted, video recording from the facility?s surveillance and additional documentation was reviewed.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
