An unannounced complaint inspection was conducted in response to a complaint that was received by licensing on 10/03/2021 regarding allegations in the area of building maintenance. The inspection was initiated and completed on 10/25/2021. On 10/25/2021, the inspector was on site from 9:30 am to 11:13 am.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. If you have any questions about this inspection, please contact the licensing inspector at (540) 430-0384.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. Large areas of rust and cracked and peeling paint were observed in two of the bathroom stalls on each side of the children's toilets.Plan of Correction: Areas in question were covered on 10-25-2021. Scraping and sanding occurred on 10-28-2021. Painting will be completed by 11-6-2021
