Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 10/25/2021 and concluded on 11/03/2021. The director was contacted by telephone to initiate the inspection. There were 46 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-260-B
Description: Based on a record review the center did not ensure an annual approval from the health department was maintained.
Evidence: The last health inspection submitted by the director during the record review was dated 01/06/2020.

Plan of Correction: The following corrective action was provided during the exit meeting:
The director has contacted the health inspector with no return calls. The director will reach out to the next person in-charge to find out the status of their health inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas of the center were maintained in safe condition.
Evidence:
Blue doors exposed to the playground had peeling paint.

Plan of Correction: The following corrective action was provided during the exit meeting:
Director will notify maintenance to repaint playground doors.
