Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews.
A monitoring inspection was initiated on 10/03/21 and concluded on 10/22/2021. The director was contacted by phone and a virtual inspection was conducted. There were 7 children present, ranging in ages from two to four, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and six staff records were reviewed.
New requirements became effective on 10/13/21. The facility has not fully complied with the following requirement; 8VAC20-780-240: The following needs to be included in orientation prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities: Prevention of sudden infant death syndrome and use of safe sleep practices; Prevention of shaken baby syndrome and abusive head trauma, including procedure to cope with crying babies or distraught children. Prevention of and response to emergencies due to food and other allergic reactions including:
a. Recognizing the symptoms of an allergic reaction;
b. Responding to allergic reaction;
c. Preventing exposure to the specific food and other substances to which the child is allergic.
The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, the center failed to get out of state sex offender registry checks before the date of hire.
evidence:
1. The inspector reviewed six staff records. Two of six records (#1 and #4) did not have a completed out of state sexual offender registry check completed before the hire date.

Plan of Correction: The director completed the checks and sent to the inspector.
