This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated as a result of an incident that was self-reported on 11/04/2021. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-270-A Description: Based on observation, a record review and interview, the center did not ensure all areas/equipment of the center were maintained in a safe condition.
Evidence:
On November 4th at about 5:45pm a parent arrived to pick-up their 2 year-old child, child A. The parent noticed child A had a screw in his mouth. After the center investigated, it was determined the screw had come loose from the play stove in the classroom.
The screw measured less than 2 inches, posing a choking hazard for 2 year-old child, APlan of Correction: The following corrective action was provided by the director:
The entire room and all equipment was looked out to ensure all pieces of equipment were in good repair. The stove was fixed and all hardware was tightened by maintenance.
