This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection resulting from a self-reported incident received by the licensing office on 8/25/2021, was initiated on 8/26/2021 and was concluded on 11/4/2021. The director was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies. One child's record was reviewed during the inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-340-A Description: Based on a review of records and interview, the licensee did not act in a way to ensure the care, protection and guidance of a child in care.
Evidence:
1. Staff #1 contacted the licensing inspector on 8/25/2021 and reported that on 7/8/2021 the parent of child #1 arrived at the center to pick up child #1 and was informed that the child was on a scheduled outing at that time.
2. Staff #1 stated that this parent was not listed as a person authorized to pick up the child from the center and that a protective order for this parent had been maintained in the child's record.
3. Staff #1 disclosed the location of the field trip when asked by child #1's parent on 7/8/2021.
4. Staff #1 stated in an interview on 8/25/2021 that she was aware that this parent was not a person authorized to pick up the child at the time that the parent requested the whereabouts of child #1 but that she gave the parent the location out of fear.
5. When the parent left the center, Staff #1 contacted the staff who were on the outing with the children to let them know not to release child #1 to the person not authorized to pick up the child. She then contacted the police and filed a report.
6. The parent went to the site of the outing and left the premises with child #1.Plan of Correction: The following plan of correction was received by the licensing office via email on 11/10/2021:
"Plan of correction
I [staff #1] would not disclose the location of the child to the parent, nor will the staff. We will
give a false location then call authorities. The staff are getting trained in Domestic Violence. We will
also use our intruder code if we ever feel we are in danger. During our yearly file check we will look for
any court documents and update"
