Technical Assistance:
An agent of the licensee currently has an expired background check. The administration is aware that the program license cannot be processed until all missing or incomplete background checks have been reviewed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A renewal inspection was initiated on 10/29/2021 and concluded on 11/3/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 46 children present during todays inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review and interview, an agent of the licensee has not complete a repeat central registry background check every five years.
Evidence: Agent #1's most recent central registry background check is dated 11/2/2016.
