Comments:
A subsidy health and safety inspection was conducted on 10/28/202. There were 14 children, ages 20 months-4 years, and 5 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, handwashing and lunch. A total of 4 children?s records, 5 staff records, and 1 manager record were reviewed. A licensing inspection was also conducted on this date. Both inspections combined cover health and safety regulations. Only subsidy requirements not cover in licensing standards were reviewed during this supplemental inspection. Information gathered during this inspection determined non-compliance with applicable standards or law and are documented on the violation noticed issued to the facility.

Description: Based on document review and discussion, the vendor failed to ensure that the written hard copy of daily attendance documents the arrival and departure of each child in care as it occurs.

Evidence: The written hard copy attendance record did not document the arrival and departure of each child as it occurred.

Plan of Correction: Will revise the form and will document arrival and departure as it occurs.

Description: Based on record review and discussion, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child 2 contained documentation of a diagnosed food allergy. There was not a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction in the record for Child 2.

Plan of Correction: Will speak with the guardian and request the information.
