63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on 10-25-21 and concluded on 11-15-21. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 53 children present, ranging in ages from 5 to 12, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of four staff files, the facility failed to insure that all background checks are conducted for each employee.
Evidence:
1.There is no out-of-state central registry clearance on file for staff #2. Hired 11-9-20

Plan of Correction: The Unit Director will insure that all background checks are conducted for all employees within the required timeframe.
