Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program (if applicable).

A 60-Day focus inspection was initiated on 11/15/2021 and concluded on 11/15/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 11 children present, ranging in ages from 3 months to 2 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirement(s): 8VAC20-780-40-K - Shaken Baby and Abusive Head Trauma written Procedures; 8VAC20-780-40-L - Staff Informed of Allergies, Sensitivities and Dietary Restrictions; 8VAC20-780-40-M - Written list of allergies, sensitivities and dietary restrictions; 8VAC20-780-60-A-8 - A written care plan for diagnosed food allergies to include a written plan from the physician; 8VAC20-780-80-A - Daily written attendance with arrival and departure; 8VAC20-780-160-A - Negative TB Screening submitted at the time of employment or before coming in contact with children and completed no more than 30 days prior to employment; 8VAC20-780-240 - Staff Orientation; 8VAC20-780-500-A - Handwashing and toileting procedures; 8VAC20-780-550 - Procedures for emergency; 8VAC20-780-560 - Nutrition and Food Services. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Violations:
Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not ensure staff wash their hands with soap and running water before and after a diaper change.
Evidence:
1. Staff #1 used hand sanitizer before and after diapering a child in the toddler classroom.
2. The center representative stated that the center staff uses hand sanitizer instead of soap and running water when cleaning their hands.

Plan of Correction: We will ensure that staff and children wash hands with soap and warm running water.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure to clean the diapering surface with soap and at least room temperature water and sanitized after each use.
Evidence:
1. After diapering a child, staff #1 wiped the diapering surface with sanitizing wipes.
2. The center representative stated that staff usually clean the diapering surface with sanitizing wipes.

Plan of Correction: It will be cleaned with soap and water and disinfectant before and after each diaper change.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure one of two staff records contained documentation of central registry results within 30 days of employment.
Evidence:
The record of staff #2 (start date: 9/14/2021) had documentation of central registry results dated 11/5/2021.

Plan of Correction: In the future if it is not received within 30 days of submission, I will make a phone call asking for results to OBI within 30 days.
