Comments:
A monitoring inspection was initiated on 11/10/2021, continued with on-site inspection on 11/15/2021 and concluded on 11/18/2021. There were 9 children present, ranging in ages from 2 years to 9 years, with 3 staff supervising and one additional office staff present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.


New requirements became effective on 10/13/21. The facility has not fully complied with the following requirements:

?8VAC20-780-40-K - written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
?8VAC20-780-40-L - inform all staff who work with children of children's allergies, sensitivities, and dietary restrictions.
?8VAC20-780-40-M - maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780- 60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
?8VAC20-780-60-A.8 - A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction
?8VAC20-780-160. Tuberculosis screening for staff and independent contractors
?8VAC20-780-240-E - Within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
?8VAC20-780-350- F - The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
?8VAC20-780-530 - At least one staff in each classroom or area where children are present shall have current first aid and CPR certification AND two staff with first aid and CPR certification must be on the premises during the center's hours of operation.

The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record reviews, the center failed to obtain a tuberculosis screening from a staff within 21 days of employment.
Evidence:
1. The inspector reviewed two staff records. One of two records reviewed (staff record #2) did not contain documentation of a tuberculosis screening and staff's date of employment was 10/04/2021.

Plan of Correction: Staff #2 is scheduled for a tuberculosis screening 11/18/2021. In the future, tuberculosis screenings will be obtained prior to employment.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have hazardous substances stored in a locked place.
Evidence:
1. The inspector observed a box containing two gallon jugs of Clorox bleach sitting in the floor in an unlocked area in the office. The inspector also observed a can of disinfectant spray and a container of Clorox wipes in an unlocked base cabinet in the kitchen area.

Plan of Correction: The cleaning supplies were moved to a locked closet during the inspection. Staff will monitor to ensure all hazardous substances are stored in a locked area.
