Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A renewal inspection was initiated on 11/15/2021 and concluded on 11/22/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 20 children, ages 3 years-5 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, physical plant, emergencies and nutrition. A total of 3 children?s records, 3 staff records and 6 Board Officer/Agent records were reviewed.

New requirements became effective on October 13, 2021. The facility has not yet complied with the following requirement(s): The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 63.2(17)-1720.1-A
Description: Based on record review and discussion with staff, the center failed to ensure that all employees shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The most recently completed search of the central registry in the record for Staff 2 was dated 06/03/2015. The record contained documentation that a repeat search was requested in 2018, however, the results of the requested 2018 search were not in the record and were not available for review during the 11/22/2021 inspection.

Plan of Correction: Submitted another request a month ago and expect to get that back soon.
