Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A subsidy inspection was initiated on 10/08/2021 and concluded on 11/10/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-580-B
Description: Based on record review and interview it was determined the center did not ensure that each staff member who work directly with children completed the required Virginia Preservice Training for Child Care staff within the first 90 days of employment.
Evidence:
1. There was no documentation of the Virginia Preservice Training in the record for Staff #1 or Staff #2.
2. Staff confirmed that this documentation was not available during the inspection.
