Comments:
An unannounced complaint inspection was conducted on 10/07/2021 from 10:20 a.m. to 11:30 a.m. in response to a complaint received by the Valley Office of Child Care Health and Safety on 09/20/2021. Four interviews were conducted with the staff, former staff, and owners, three staff records, policies and procedures were reviewed. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". The exceptions to compliance are noted on the violation notice.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22VAC40-185-340-F
Complaint related: Yes
Description: Based on observation, the center failed to ensure sight and sound supervision of children at all times.

Evidence:
1. On 10/07/2021 at approximately 11:05 A.M., ten two year olds were left without supervision in the classroom for approximately 45 seconds.
2. Staff #2 was in the restroom with four children assisting with handwashing. Staff #1 was left in the classroom with ten children. Nine children were playing various centers, and one child was sitting at the table drinking water. Staff #1 left the classroom and went into the kitchen and closed the door. The kitchen is adjacent to the classroom.
3. Staff #2 did not communicate with Staff #1 that she was leaving the classroom. The Licensing Inspector was the only adult in the classroom with ten two year olds for approximately 45 seconds.

Complaint related: Yes
Description: Based on review and interview, children of three years of age to age of eligibility to attend public school (five years by September 30), were not in ratio of one staff member to every 10 children.

Evidence:
1. On the date of the investigation (10/07/2021), at approximately 10:20 a.m. there were 16 children being supervised by one staff member.
2. Staff B confirmed that the classroom was out of ratio with 16 children to one staff member.

Standard #: 22VAC40-185-500-B
Complaint related: Yes
Description: Based on observation and interview, the center failed to ensure that proper cleaning procedures were followed after diapering a child.

Evidence:
1. After changing a child Staff #2 first sprayed bleach water onto the surface. Staff #2 then turned and washed her hands and returned to the changing table. She took a disposable towel and wiped the surface. Staff #2 then started to spray the table with a soap water solution.
2. Staff #2 stated she was trained to spray the changing table with bleach water, and then wash her hands and the child's hands. After handwashing is complete, return to the diaper changing area and wipe the surface and spray with soap and water and wipe the surface dry.
3. Th proper procedure is to clean the surface with soap and water and wipe dry, and then to follow with a sanitizing agent and let it air dry. When using bleach water two minutes air dry time is recommended before wiping the surface dry.
