An unannounced complaint investigation was jointly investigated with a local government agency on November 22, 2021. Staff Interviews and parent interviews were conducted, records reviewed, and observations made in response to a complaint alleging forbidden actions. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid. During the investigation, violations were determined pertaining to children records, immunizations, and physical examinations. Standard #: 22VAC40-185-130-A Complaint related: No Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #1's record (Start date 09/05/2021) did not have documentation of immunizations in the record.Plan of Correction: The child has withdrawn from the center, but the center will ensure that all children will have documentation of immunizations in the record before the first day of attendance.
Standard #: 22VAC40-185-140-A Complaint related: No Description: Based on record review, each child did not have a physical examination by or under the direction of a physician.

1. Child # 1's record (Start date-09/05/2021) did not have documentation of a completed physical examination.Plan of Correction: The child has withdrawn from care, but the center will ensure that each child has a physical examination in the record before the child's first day of attendance.
