Thank you for your assistance during this unannounced inspection. This inspection was initiated by complaints that were received by our licensing office on August 5, 2021 and August 9, 2021, regarding the allegation of staff supervision. The on-site visit was conducted on September 21, 2021 from 9:39 am through 11:18 am. There were eighty-six children in the care of nine staff at the inspector's arrival and twelve staff at the inspector's departure. The evidence gathered during this investigation supports the allegation, therefore the complaint is valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-350-E-1 Complaint related: Yes Description: Based on observations and interviews, the center did not ensure that the staff to child ratio for children from birth to the age of 16 months did not exceed one staff member for every four children.
Evidence:
1. At 9:39 am on 9/21/2021, the director approached the front door of the center with her classroom following behind. The inspector was informed that the owner did not feel comfortable with the inspector going around the center unescorted, so it would be about an hour before additional staff would arrive and entry allowed.
2. Staff #1 arrived at 10:20 am. Staff #2 arrived at 10:30 am. Staff #3 arrived at 10:44 am. The inspector was allowed entry once Staff #2 arrived. The director and her classroom escorted the inspector around after the arrival of Staff #3.
3. There were eleven children under the supervision of two staff in the Monkeys (Infants: 10 ? 12 months old) classroom at the time that the census was checked shortly after 10:44am. No children arrived or departed between the time the inspector arrived at 9:39 am and the time that the census was checked.
4. There were fifteen children under the supervision of two staff, including Staff #3, in the Alligators (Infants & Toddlers: 15 -24 months old) classroom at the time that the census was checked shortly after 10:44 am. No children arrived or departed between the time the inspector arrived at 9:39 am and the time that the census was checked. When children of mixed age groups are combined, the staff-to-child ratio for the youngest child in the group applies to the entire group.

Evidence:

Standard #: 22VAC40-185-350-E-4 Complaint related: Yes Description: Based on observations and interviews, the center did not ensure that the staff to child ratio for children from three years to the age of eligibility to attend public school did not exceed one staff member for every 10 children.
Evidence:
1. At 9:39 am on 9/21/2021, the director approached the front door of the center with her classroom following behind. The inspector was informed that the owner did not feel comfortable with the inspector going around the center unescorted, so it would be about an hour before additional staff would arrive and entry allowed.
2. Staff #1 arrived at 10:20 am. Staff #2 arrived at 10:30 am. Staff #3 arrived at 10:44 am. The inspector was allowed entry once Staff #2 arrived. The director and her classroom escorted the inspector around after the arrival of Staff #3.
