This inspection consisted of a virtual component and an in-person component. Information gathered during both components determined that there were (2) non-compliances with applicable standards. These violations were documented on the Violation Notice issued to the program.
- Comments:

The in-person component of the inspection took place between the hours of 10 am through 11:30 am on 11/30/2021. The LI met with the Staff of Heights Academy Preschool located in Chantilly, Virginia. At the start of this unannounced renewal inspection there were a total of 6 children (mixed age group with the youngest child 2 years of age) observed with 2 staff; good ratio in this 1 room. A total of 6 children?s files and 3 staff files were reviewed. The children were engaged in the following: outside play, games, music, song, story time, free play, diapering, and other organized activities. Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 6 children files indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after attendance.
Evidence -
1. On the date of inspection (11/30/2021), documentation of a physical examination was not available for review for Child #4.
2. Based on the information on file, the start date for Child #4 was listed as 09/07/2021.Plan of Correction: A message was left today for this child's parent. This documentation will be received within the next two weeks, if not tomorrow.
Standard #: 22VAC40-185-330-A Description: Based on observation, the playground contains several tripping hazards.
Evidence -
1. On the date of inspection (11/30/2021), the black cloth that was laid down on the playground to prevent grass from growing was now visible; poking up through the resilient surfacing.
2. Several folds of black cloth appear around the playground and near climbing equipment. These places where the cloth was observed pose as possible tripping hazards as the children run through the playground.Plan of Correction: A new layer of mulch will be ordered and placed on the playground to cover these areas. In the meantime, children will use the grass field for games and play.
