Comments:
An unannounced inspection was conducted in response to a complaint that was received by the department on 10/26/2021 regarding allegations in the area of staff to child ratio requirements and daily activities for infants. The inspection was initiated on site on 11/4/2021 from 7:33 am to 8:30 am.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. There was also a violation found during the inspection regarding hazardous substances. See the violation notice for more details. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Cleaning chemicals were observed in an unlocked classroom. The disinfecting wipes contained the warnings: hazard to humans, and the alcohol wipes contained the warnings: flammable and keep out of the reach of children.
2. Administration acknowledged that the classroom was unlocked.

Description: Based on observation and interview, the center did not ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group.

Evidence:
1. The licensing inspector arrived on site at 7:33 am. At 7:40 am, one staff was observed with 11 children ranging in ages from 2 to 9 years in the preschool gathering room. At 7:48 am one staff was observed with 19 children ranging in ages from 2-9 years. At 7:58 am, two staff were observed with 24 children ranging from ages 2-9 years. At 8:00 am, two additional staff arrived and the ratio was able to be maintained.
2. Staff were interviewed and acknowledged that there have been mornings where ratio could not be maintained in the gathering room due to staff calls outs or unexpected changes in child arrival times. Administration stated that they were reviewing arrival times and making necessary adjustments to the schedule so ratio could be maintained.
