Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on December 3, 2021 and concluded on December 3, 2021. There were 27 children present with 8 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on record review, staff was not evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening was not submitted at the time of employment and prior to coming into contact with children.

Evidence:
1. Staff #2's record (Date of hire-09/21/2021) contained documentation of a TB test results dated 09/24/2021. The TB results were obtained after the date of hire.

Plan of Correction: Staff will complete TB testing prior to employment.

Standard #: 22VAC40-665-540-B
Description: Based on record review, the center did not obtain subsequent TB screenings at least every two years from the date of the initial screening, or more frequently if recommended by a physician.
Evidence:
1. Staff #1's record (Date of hire-09/28/2020) contained documentation of an expired tuberculosis screening dated 09/18/2019. Documentation of a subsequent tuberculosis screening was not available for review.

Plan of Correction: Staff will complete a new TB screening and the record will be documented.

Standard #: 22VAC40-665-580-B
Description: Based on record review, Within the first 90 days of employment all staff who work directly with children did not complete Virginia Preservice Training for Child Care Staff.
Evidence:
1. Staff #3's record (Date of hire 08/30/2021) did not contain documentation of the Virginia Preservice Training for Child Care Staff. At the time of the inspection, documentation was not available.

Plan of Correction: Staff will complete the Preservice training and the record will be documented.
