This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated on 12/7/2021 and concluded on 12/9/2021. There were 66 children present with 13 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-580-B Description: Based on record review and interview, the vendor did not ensure to obtain documentation of the completion of the Virginia Preservice Training for Child Care Staff Within the first 90 days of employment for one staff as required.
Evidence:
1. The record of staff #4 (DOH 6/14/2021) did not contain documentation of the preservice training.
2. Administration acknowledged that it had not been completed.Plan of Correction: Staff will complete Preservice training within 90 days of employment. Teacher #4 is in process of completing Preservice 10 hr. Has completed 3 hr.
Standard #: 22VAC40-665-580-E-1 Description: Based on record review and interview, the vendor did not ensure to obtain current CPR certification appropriate to the ages of children in care within the first 90 days of employment for one staff as required.
Evidence:
1. The record of staff #4 (DOH: 6/14/2021) did not contain documentation of CPR certification.
2. Administration acknowledged that the staff member did not have a current certification.Plan of Correction: Staff will complete CPR within 90 days of employment. Teacher #4 is scheduled to take CPR. (online portion completed)
