Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A non-mandated monitoring inspection was conducted on November 18, 2021. The director was contacted by telephone and the inspection was conducted. The inspection was the result of a self-reported incident that occurred on November 17, 2021. There were 43 children present and 57 children enrolled. The inspector reviewed compliance in the areas of administration, staff qualifications, staffing and supervision, and special services. A total of 2 children's records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interviews with the director and two staff members and verification from staff, the center failed to ensure the care and protection of one child by giving the child the wrong breastmilk.

Evidence:
1. Staff Member #1 reported that on 11/17/2021, in Classroom A, she removed a bottle of breastmilk labeled with Child A's name from the refrigerator and put it in the bottle warmer. She then picked up Child B (age 7 months), changed her diaper and placed her on a Boppy on the floor. She removed the bottle from the warmer and handed it to Child B, who holds her own bottle. Child B began drinking from the bottle. When Staff Member #1 went back to the refrigerator to get a bottle for Child A, she realized that she had given Child B the wrong bottle of breastmilk. Child A's bottle of breastmilk held 5 ounces, and Child B drank 2 ounces before Staff Member #1 realized her mistake.
2. Staff Member #1 reported that she failed to read the label on Child A's bottle before putting it in the warmer or before giving it to Child B.
3. Staff Member #2 was in Classroom A with Staff Member #1. She stated that she did not see Staff Member #1 get the bottle out of the refrigerator, warm it or give it to Child B. Staff Member #1 told Staff Member #2 of the mistake and Staff Member #2 told her that she needed to report it to the director.
4. The director reported that Staff Member #1 came to her and told her that she had given Child B another child's breastmilk.

Plan of Correction: The director will continue to train Staff Member #1 as an infant teacher and have all staff read and sign for acknowledgement, a document called Safe Bottle Feeding. Management will be available to answer any questions they have regarding the document. The document has also been posted in all classrooms in a visible area and the procedures are being implemented in all classrooms.
