22.1 Early Childhood Care and Education

Comments:
An unannounced renewal inspection was conducted today from 8:55am-11:05am. There were 26 children directly supervised by 7 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No medications were on site. Children were observed participating in circle time, engaging in free play, building with blocks, finishing breakfast, and working on a holiday craft project. Positive interactions between children and staff were observed during this inspection. Areas of non-compliance are identified in the Violation Notice.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on Monday 12/20/2021. Please email me at morgan.bryson@doe.virginia.gov with any questions.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the center did not ensure that all staff submit to a fingerprint based criminal history background check prior to employment.

Evidence: Staff #4 stated that Staff #1 (Date of Hire: 8/16/2021) is a rehire and updated fingerprints were not completed prior to rehiring Staff #3.

Plan of Correction: Fingerprints were done on 12/11. Waiting on results.

Standard #: 22.1-289.035-B-3
Description: Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry search within 30 days of employment.

Evidence: Staff #4 stated that Staff #1 (Date of Hire: 8/16/2021) is a rehire and an updated Central Registry search was not conducted within 30 days of the staff's rehire date.

Plan of Correction: This was mailed to state on 12/8/21. Waiting on results.

Standard #: 8VAC20-780-80-A
Description: Based on review of attendance records, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child as it occurs.

Evidence: At approximately 9:05am, there were two children present in the Toddler A classroom and only one child was signed in on the classroom attendance sheet. Staff #2 advised that the child who was not signed in has been there since approximately 7:50am this morning.

Plan of Correction: Child was written in our attendance sheet once this was noticed. Spoke to staff about making sure all children are signed in/out as they come in or leave daily.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the playground area, areas and equipment outside the center were not maintained in a safe condition.

Evidence: 1. The playground surfacing fabric at the base of the slide on the smaller climbing structure has split and there was frayed fabric sticking up posing a tripping hazard. 2. There is a small hole of cracked and jagged plastic at the bottom of the larger climbing structure that could cause fingers or hands to get stuck

Plan of Correction: Has been on maintenance list for some time. Needs to be fixed ASAP. I have reported the issue again as of 12/7/21.

Standard #: 8VAC20-780-550-E
Description: Based on record review and staff interview, the center has not ensured that shelter in place procedures are practiced a minimum of twice per year.

Evidence: The two most recent shelter in place drills were conducted 12/2/2020 and 12/3/2019.

Plan of Correction: While on leave staff did not do shelter in place drill. Will maintain moving forward.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child's name.

Evidence. One bottle of prepared infant formula was observed in the refrigerator in the Infant A classroom that was not dated or labeled with the child's name.

Plan of Correction: The teacher labeled the bottle. Spoke to teacher to check all bottles daily at drop off to prevent this from happening again.
