This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated on November 4, 2021 and concluded on November 4, 2021. There were 71 children present with 16 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-665-540-A Description: Based on a review of staff records and interview, the vendor did not ensure that each staff submitted documentation of the tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: 1. The record of Staff #4 (DOH 8/30/21) contained documentation of a tuberculosis test dated 9/5/21. 2. Administration acknowledged the tb was late.Plan of Correction: To ensure all new staff members have TB completed before they start with us on their first day.
